Executive Safety Dashboard Template

Most safety reports tell leaders how last quarter went. This free Excel template helps you show where next week's risk is building. Enter a handful of leading indicators each week for every site, crew or contractor, and the template calculates a 0 to 100 risk score, flags who moved and why, and rolls everything into a board-ready summary with leading and lagging indicators side by side.

How to use the executive safety dashboard template

1. Add your sites, crews and contractors

List every unit you want to report on in the Settings tab, with its region. Include contractors. They often carry a large share of driving and field risk.

2. Review the targets and weights

The template comes with sensible defaults for each leading indicator. Adjust the targets to match your program, and give more weight to the indicators that matter most for your operation.

3. Enter one row per unit each week

Add counts such as overdue actions, inspections scheduled and completed, near-misses, observations, missed check-ins and fatigue flags. Leave an indicator blank if you don't track it yet and it's simply left out of that week's score.

4. Review the dashboard every Monday

Start with units in red and anything that dropped five or more points, then check which indicators moved and who owns the fix.

5. Report to the board

Set the reporting period, add your commentary and paste the Board Summary into your board pack.

What is an executive safety dashboard?

An executive safety dashboard is a short, regular view of safety performance built for leaders rather than safety specialists. Instead of dozens of charts, it shows a few leading indicators, how they are trending and where leadership attention is needed. Good executive dashboards put leading indicators, such as overdue actions and inspection completion, next to lagging indicators, such as recordable injuries, so the board can see both where risk is building and how outcomes are tracking.

Which leading indicators does the template track?

The template tracks eight leading indicators chosen for field-heavy operations: overdue corrective actions, inspection and audit completion, near-misses and hazards reported, safety observations, missed or late check-ins, journey escalations and SOS events, fatigue flags, and high-risk tasks completed with a permit or risk assessment. How is the weekly risk score calculated?

Each indicator is scored from 0 to 100: 100 when it meets your target and 0 at the level you set as unacceptable, with values in between scored proportionally. The weekly risk score is the weighted average of the indicators you reported that week. By default, 80 and above is green, 65 to 79 is amber and below 65 is red. You can change all of these on the Settings tab.

When should you move beyond a spreadsheet?

A spreadsheet is a good way to prove which indicators drive action. It gets harder as you add sites and contractors, because someone has to collect the numbers by hand every week and they often arrive too late to act on.
